Fit Notes

If you require a Medical Fit Note from your GP, you may have to arrange an initial appointment to discuss the nature of your request.  Some hospital admissions and subsequent discharges may be obvious and an initial appointment might not be required, i.e. broken bones.  Please note in these circumstances, your GP is unable to issue a fit note until your clinical notes have been received from relevant health care professional.

For repeat medical fit notes, you must phone the Practice on the day that it is due.  The Practice is unable to issue fit notes earlier.  The GP may refuse a repeat fit note and request instead that a review appointment is made to discuss your situation further.

For your first 7 days of sickness/ill health you should complete a self certificate which can be found at www.gov.uk – this must be completed online.  A GP can not provide you with a fit note for the first 7 days of your absence.

You should not make an appointment for a repeat fit note – this request should be made over the telephone by calling 0141 531 6010 on the day it is due or by emailing us Lan.clinicalovertounmedicalpractice.co.uk
